Terms & Conditions

  1. Validity: This Health Screening Gift Card (the "Card") is valid for single use only and expires one year from the date of purchase.

  2. Redemption: This card can be redeemed only at Parkway MediCentre ("PMC"), located at The Woodleigh Mall, 11 Bidadari Park Drive, #02-25/26/27, Singapore 367803.

  3. Recipient: This Card is transferrable and the value of card has been determined upon sale and cannot be changed.

  4. Pre-arranged appointment: Appointment is compulsory prior to the redemption of the service. Please contact PMC at 6229 6868 or via WhatsApp at 9012 1569 with the following information to make an appointment:
    1. Preferred Health Screening date and time. Fasting is required.
    2. Name, NRIC/Passport Number and Contact Number
    3. Card Reference Number
    4. Purchaser's Name (For Verification Purposes)

  5. Presentation: Please bring along this Card on the day of the health screening appointment at PMC. Failure to present the Card at the time of redemption may result in denial of service.

  6. Value: The value of the Card is predetermined and cannot be exchanged for cash. No refunds or credits will be provided for any unused portion of the Card value. It cannot be shared or combined with any other offers or promotions.

  7. Lost or Stolen Cards: PMC is not responsible for the lost, stolen, damaged, or unauthorized use of the Card. Lost or stolen cards will not be replaced or refunded.

  8. Expiration: The Card expires on the date indicated on the sticker at the bottom of the card. After this date, the Card will be voided, and cannot be redeemed nor extended.

  9. Use Restrictions: The Card cannot be used for any purpose other than the designated health screening product specified by the issuer. It cannot be used to purchase other products or services offered by the issuer or any third-party vendors.

  10. Limitation of Liability: The issuer shall not be liable for any damages, including but not limited to incidental, consequential, or punitive damages, arising out of the use or inability to use the Card.

  11. Governing Law: These terms and conditions shall be governed by and construed in accordance with the laws of Singapore, without regard to its conflict of law provisions.

  12. Modification and Termination: The issuer reserves the right to modify and terminate these terms and conditions at any time without any prior notice. Any such changes will be effective immediately upon posting on the issuer's website or other communication channels.

  13. Acceptance of Terms: By purchasing, redeeming, or attempting to redeem the Card, the cardholder agrees to be bound by these terms and conditions.
